A passenger from USA aircraft with 64 individuals on board with him plunged into the Potomac River following a midair collision with a US Army Black Hawk helicopter engaged in a nighttime training exercise. Fewer than 30 seconds prior to the collision, an air traffic controller at Reagan National Airport had a conversation with the military helicopter: "PAT25, do you have the CRJ in sight?"  Afterwards, the controller issued another instruction: "PAT 25 pass behind the CRJ." The fire chief said, "Officials don't 'believe there are any survivors' in Washington plane crash." There was no response from the helicopter, and a few  moments later, the 2 aircraft collided. A pilot witnessing the moment urgently radioed, "Tower, did you see that?" The final conversations between the crew members of the military helicopter were captured by audio from LiveATC.net, a repository for in-flight recordings.

"Crash, crash, crash, this is an alert three," one of the air traffic controllers can be heard saying this, as reported by news agency Reuters.

American Airlines, whose regional subsidiary PSA Airlines operated the Bombardier CRJ-700, gave confirmation  that 60 passengers and 4 crew members were aboard the aircraft. The US Army indicated that a helicopter was involved in a "training flight" during the time of the collision, and the Black Hawk was carrying 3 soldiers.
